General Information
- New!! The Annual Permission Form is now available to all troops and is approved as an option for obtaining permission for girls to participate in local trips which are located within one hour's driving time of the regular meeting place, do not exceed 6 hours and are not considered high-risk as outlined in Safety-Wise. To use this form, leaders must agree to communicate the details regarding these trips to the parents and the trip approval coordinator and to carry the Annual Permission forms along with the girls' Health History forms at all times.
- A new Health History form has also been created with annually updated signature lines so that parents can review the information and if nothing has changed simply re-sign and date the form.

Required Forms & Notifications
Please see below for the different kinds of trips possible, and the required forms and/or notification needed. For any questions regarding trip approvals, email tripapprovals@redwoodcitygirlscouts.org.
- Change of usual location, date, and/or time of regular meeting – one page permission slip, or annual permission slip. Must submit a copy of the permission slip or complete the simple trip web-form and send to trip approval coordinator one week before the meeting.
- Local field trip in lieu of your troop meeting – one page permission slip or annual permission slip. Must submit a copy of permission slip complete web-form and send to trip approval coordinator one week before the meeting.
- One day trip inside local boundaries – one page permission slip or annual permission slip. Must submit a copy of one page permission slip or complete web-form and send to trip approval coordinator one week before the meeting.
- Overnight trip (2 nights or less) – one page permission slip to troop parents, AND ‘Trip and High Risk Approval Form’ to trip approval coordinator 2 weeks before the trip.
- Overnight trip (3 nights or more) – one page permission slip to troop parents, AND Trip and High Risk Approval Form to trip approval coordinator 4 weeks before the trip.
- Any trip outside the local boundaries – one page permission slip to troop parents, AND Trip and High Risk Approval Form to trip approval coordinator 4 weeks before the trip.
- Any high risk trip or activity* [see Safety Wise for list of activities considered to be high risk] -one page permission slip to troop parents, AND Trip and High Risk Approval Form to trip approval coordinator 4 weeks before the trip or activity.
- International trip – one page permission slip to troop parents, AND Trip and High Risk Approval Form to trip approval coordinator 1 year before the trip.
Please note that if you are using the new annual permission slip in lieu of the one page permission slip you can simply notify the trip approval coordinator of your troop’s local trips by submitting this form via fax it at 650 204-6851 or send me all of the requested info by email: tripapprovals@redwoodcitygirlscouts.org.
* High Risk Activities - Any activity that has a high potential for injury and/or that requires special skills, training, and equipment.
